I hereby make formal application to enter the South Island Surfing Championships and to abide by the decision of the judges and organisers. I agree to abide by the event rules and am fully aware that I am entering this event at my own risk. I hereby release the sponsors, their members or agents and any officials connected with this event from all liability for injuries or damages whatsoever arising from my participation or presence at this event. I also agree to allow the use of any moving or still photographic images of me for the purpose of promotion as deemed necessary by contest organisers.
